CURRENT ORIGINAL DOCUMENTS TO REQUEST OR RENEW IRS NUMBERS (ITIN NUMBER) FOR TAXPAYER AND SPOUSE IN USA.


TAXPAYER OR SPOUSE:

  • VALID PASSPORT OR VALID ID WITH PHOTO.
  • BIRTH CERTIFICATE


FOR CLIENTS WITH A W-2 UNDER THEIR NAME VALID ORIGINAL DOCUMENTS TO REQUEST OR RENEW ITIN NUMBER FOR DEPENDENTS LIVING IN THE US.


ORIGINAL PASSPORT (VALID AND SIGNED) IS THE ONLY DOCUMENT THAT CAN BE PRESENTED IF YOU HAVE A DATE OF ENTRY INTO THE USA. IF YOU DON’T HAVE A PASSPORT OR A DATE OF ENTRY, YOU MAY BRING THE FOLLOWING DEPENDING ON YOUR CASE:


ADULTS (18 AND UP):

  • Valid ID with photo.
  • Birth Certificate
  • You can present one of the following documents: rent contract, utility bill (such as water, electricity), or bank statement that shows the name and address in the US of the applicant.
  • (If passport with entry into the US is not presented)

MINORS WHO GO TO SCHOOL (FOR CHILDREN 17 AND YOUNGER):

  • Birth Certificate.
  • Proof of study: the document must have the date, the name of the student, date of birth, grades that were obtained included in the document or separate, grade, in addition to the official school logo or stamp, signature, phone number, and the address of the school (the date of preparation CANNOT exceed more than a year)

MINORS WHO DO NOT GO TO SCHOOL (FOR CHILDREN 6 AND YOUNGER):

  • Birth Certificate.
  • Immunization and medical records (accepted only if they show the name of the applicant and the chronological order of the medical care that was received)
  • Doctor’s note: must show the name, the date of birth, and the address of the child. Must also have the logo, stamps, and signatures in addition to the name, address, and phone number of the medic, hospital, or clinic where the child attends (The date of preparation CANNOT exceed more than a year)
